How to request verification of enrolment for a Service Canada (CPP) - Declaration of Attendance聽at School or University.
Instructions
- Do听苍辞迟听mail or drop off your Service Canada form (ISP-1401) to Camosun as the college does not stamp or sign the form. Service Canada will accept a proof of enrolment document (Camosun Verification聽of Enrolment) to confirm your attendance.
- After your first day of classes, obtain聽a聽Verification of Enrolment听迟丑谤辞耻驳丑听.听
- See the mailing instructions on the last page of the Service Canada form (ISP-1401).
- Important: Do not complete Section C on the Service Canada form.
- Submit your Verification of Enrolment along with the Service Canada 鈥 CPP forms to Service Canada. Camosun does not mail or email letters or forms on the student鈥檚 behalf.
